Insurance & Billing

Pediatric Associates of Greater Salem accepts most major managed care (HMO, PPO), private indemnity, and Medicaid subsidized plans. Call your insurer to verify that we participate in your plan.

Bring your insurance card to every visit. We do our best to verify your eligibility and coverage. However, it is your responsibility to know your plan's benefits and restrictions and to inform us of any changes in your plan.

Insurance & Billing FAQs

Q. Do I need my card at each visit?
A. Yes. We may request your card to update or verify information.

Q. Is there a co-payment at each visit?
A. Yes. We require a co-payment for the visit at the time of service for physicals, follow-ups, consults, and sick calls.

Q. Are immunizations and blood tests covered by my insurance company?
A. It depends on the insurer. Call your insurance company for an answer.

Q. Do I have to wait a full calendar year between physicals to be covered for the visit?
A. Most insurance companies will cover a physical visit only if 365 days have passed since the last physical appointment. Call your insurance company to inquire.

Q. How long after my visit will I receive a statement?
A. We bill every 30 days, but it is likely you will receive a statement soon after the visit.

Q. What happens if I change my insurance company?
A. You must choose a primary care provider (PCP) from our practice before the visit, and inform our office of the change of insurance.

Q. Do I need to register my newborn on our insurance plan?
A. Yes. Be sure to choose a primary care provider from our practice.

Q. How do I file a claim for an auto or work-related accident?
A. Provide us with either the auto insurance or workman's compensation form, with all pertinent information included.

Q. Do all manage-care policies require the subscriber to call for a referral?
A. Yes. Otherwise, check with your insurer.
